Requirements for Helpers

Many of our parish activities and organizations--especially those which involve youth and vulnerable individuals--require VIRTUS Training. If you have yet to complete your training and verification, please do so now. 

Disciples serving at Holy Family Catholic Church and Holy Family Academy must fulfill certain requirements before they begin:

Click here if you recently had a background check done at ANOTHER PARISH

Click here if you are a CURRENT EMPLOYEE or RENEWING your Enhanced Essential 3 (VIRTUS/Background Check & Code of Conduct)

ALL Teen Disciples (ages 13-17) must have a Code of Conduct and TWO References on file:

- Once you have completed all of the forms you may send them to Mary Boyle via email or drop them off at the parish office in a sealed envelope addressed to: Mary Boyle - CONFIDENTIAL
- All teens are also required to complete personal safety lessons (these are provided to classes at HFA, the Youth Group, those in Religious Education classes, and Chesterton Academy).  Please contact Mary Boyle at 952.929.0113 for more details, as well as to verify once you have taken the lessons.
